BCFO provides Lymphedema garments through area medical supply providers. To qualify, candidates must be uninsured or underinsured, and lymphedema must be a result of breast cancer treatment.
Process
- Fill out the Lymphedema Garment application and Medical Records Authorization and fax, mail or drop off at BCFO office: BCFO, 330 N Jefferson, Suite B, Springfield, MO 65806
- Please be specific about what type of garments are needed.
- Applicants will receive a phone call to confirm receipt of application and to obtain a short case history.
- BCFO will request a copy of the most recent medical records related to your need for lymphedema garments
- Once all necessary information has been received, applicants will receive a letter regarding approval with instructions on how to proceed.
Payment for Lymphedema garments will be paid to invoice to the garment provider.
